| Description: | Serves as a BCCS Case Manager following the criteria set by the BCCS program which includes documentation, problem solving, identifying needs or problems of clients, determination of eligibility and accessibility. Coordinates program services with providers, medical support staff, and community partners. |
| Qualifications: | Graduate from an accredited high school or GED program. Graduate from a four year college or university in Public Health, Social Work, Sociology, Psychology, Health Education or a related field, with at least one year experience in a health education, training, or research capacity. OR five years experience preferably in a community health care setting.
